Terms and conditions for FIT4MOM Piedmont Membership

MEMBERSHIP AGREEMENT

This contract made between the client and FIT4MOM Piedmont is a 3 month agreement (unless stated otherwise) and except as provided in the initial 10 day cancellation policy below, may be terminated by either party with the proper advance notice. This membership allows you to attend Stroller Strides/Barre classes per week at any location.

FIT4MOM OnDemand Monthly Subscription

With your paid monthly membership you are given free access to FIT4MOM OnDemand. You will have unlimited access to OnDemand workouts. When your FIT4MOM membership is properly cancelled according to the Terms and Conditions, your OnDemand subscription will be automatically cancelled. If you would like to keep your OnDemand subscription you can re-enroll using the same login information and purchase your desired paid subscription.

AUTHORIZATION FOR PAYMENT
I authorize FIT4MOM Piedmont to initiate a charge to the credit card number entered into Pike13 for my dues. This authorization is to remain effective until I have cancelled my membership through the FIT4MOM franchise owner.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with Stroller Strides to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a return fee of $25. Members are responsible for updating their card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.

CANCELLATION POLICY
To cancel this membership, the member must contact the FIT4MOM franchise owner prior to their next payment due date to request a cancellation. No refunds will be given for dues already paid. This membership is not transferable to another person or anywhere outside the Piedmont franchise. Three month minimum commitment. Membership continues after minimum commitment is met. $75 early cancellation fee. Once a membership is cancelled, if the member wants to re-enroll they will be charged the full registration fee.

HOLD POLICY
15 day notice is required for membership cancellation. To cancel, email Christine Magee at Christinemagee@fit4mom.com. Your hold date will be set for 30 days after your emailed request. Maternity Leave Holds are handled on a case by case basis. Account will be charged $25 per month, up to 2 months twice a year.

INITIAL 10-DAY CANCELLATION POLICY
You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement, written notice must be received by FIT4MOM Piedmont within the initial ten days. You are responsible for paying for the classes already attended.

By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice.
